“Activate your Patient Portal and Complete Application.”
Completing New Patient Paperwork:
- Complete your new patient forms using the Patient Portal. These forms are very important to our clinical process.
- To avoid having to reschedule, please make sure that your forms are completed 2 days prior to your Intake Visit.
